How does a person supported start using the Flexible Residential Support Model?

If a person is eligible for Supported Living or Residential Habilitation Levels 2 through 4 and is interested in spending more time independently in their home, they can talk with their Circle of Support to learn more about the Flexible Residential Support Model and request for it to be added to their service plan.

What requirements must be met for Plans Review to approve the Flexible Residential Support Model?

After a person’s Circle of Support requests that the Flexible Residential Support Model be added to someone’s service plan, Plans Review will make sure all of the following requirements are met: • The person supported is eligible for Supported Living or Residential Habilitation Levels 2 through 4. • The person supported has a staffing plan that describes how their support needs will be met when staff are not physically present in the home with a combination of: Enabling Technology, Remote Support Services, and/or Natural Supports. • The person supported has an emergency response plan that describes how agency staff will respond in the event of an emergency or other urgent situation requiring immediate attention.

Is a person supported required to use the Flexible Residential Support Model?

No, a person supported is not required to use the Flexible Residential Support Model. A person eligible for Supported Living or Residential Habilitation Levels 2 through 4 can choose between the traditional residential model or the Flexible Residential Support Model.

Is there a minimum or maximum number of hours on a given day that a person can use the Flexible Residential Support Model?

No, there is not a set minimum or maximum number of daily hours a person can use the Flexible Residential Support Rate. Their daily and weekly schedule can be customized to meet their needs and preferences.

If someone is over their waiver cap, can they choose to use the Flexible Residential Support Model?

If a person is over their cap and is already receiving a Supported Living or Residential Habilitation service, the Flexible Residential Support Model can be approved to be used under that service at the same Level of Need. If a person is over their cap, tries the Flexible Residential Support Model, and decides to go back to using a traditional residential service, it can be approved at the same Level of Need.
